As a service to you, our office will prepare and submit all insurance claims for you. However, we do consider the patient primarily responsible for the account. Please present your insurance identification card at the time of your visit.

We are participating Medicare physicians. All claims will be filed for you. You will be responsible for your annual deductible, and the 20% Medicare does not pay. If you have Medicare supplement insurance, we will file a claim to them for the 20% balance as a courtesy to you.

We also accept Blue Cross, Health Choice, MedNet, Complete Health, Viva Health Care and most other Insurance plans. We ask that you pay your co-pay amounts at the time of service.

If you do not have insurance you will need to make financial arrangements with our office or accounts receivable manager before elective surgery may be scheduled.

Office hours are Monday through Friday from 8:30 a.m. to 5:30 p.m. A pre-recorded message will instruct you when the office is closed.

Call 734-7850. To enable a better selection of time and date, please call for appointments as early as possible. If you are unable to keep an appointment, please notify us 24 hours in advance. This will provide the opportunity for another patient in need of care to be seen.

Emergencies: If you need to consult with a doctor after regular office hours, you may call the hospital operator. She will assist you in reaching a doctor.

Our patients are hospitalized at either Cullman Regional Medical Center (256)737-2000 or Woodland Medical Center (256)739-3500. One Day Surgery procedures are performed at each hospital.

We appreciate and encourage payment for services at the time of your visit. If you have any questions regarding our fees, our office manager or accounts receivable manager are available to assist you with financial arrangements. For your convenience, Master Card and Visa are accepted.
